Job Description:
Colorado Visiting Nurse Association has an immediate need for an
Occupational Therapist. Sign-on Bonus of $3500!LOCATION: Southwest
Denver MetroSTATUS: Full-timeSCHEDULE: Monday-Friday, and weekends
every 4-6 weeksPay is based off a Per Visit Rate scale.SUPPLEMENTAL
PAY: Based on position, schedule and/or availability: Paid Mileage,
Shift Differentials, Stipends, OvertimeMILEAGE AND EXPENSE
REIMBURSEMENT: Sixty-two and a half cents per mile one of the

ProgramsREWARDING WORK YOU WILL DO:Responsible for performing
occupational therapy, evaluating patient needs, and conducting case
and resource management.Provides initial and ongoing assessments to
determine level of function, including OASIS assessments at
appropriate time points.The procedures required by the therapist
may include (but are not limited to): muscle re- education,
ADL/IADL/MRADL training, adaptive equipment, orthotics/prosthetic,
fine motor coordination, sensory rehabilitation, therapeutic
exercise, energy conservation training, functional balance support
training, work-hardening concepts, and cognitive
assessments/interventions.Advises and consults with the family,
physician and other agency personnel, including discharge
planning.Other duties and responsibilities as assigned.WHAT WE WILL
LOVE ABOUT YOU:Minimum Education: Graduation from a college or
university, with a degree in Occupational Therapy.Minimum
Experience: One year of experience as an occupational therapist
required, with two years of SNF, hospital or inpatient rehab
preferable. Home health experience also preferred. Other
requirements may vary by case or department needs.Required License:
Active Colorado OT license. Current CPR certification based on
individual affiliate requirements. Valid Colorado drivers license
and proof of automobile insurance in accordance with state
statute.PHYSICAL REQUIREMENTS:Ability to lift/push/pull minimum of
35 lbsColorado Visiting Nurse Association is affiliated with Care

palliative care organizations serving Colorados Front Range and the
Regions Largest Home Health, Hospice, and Palliative Care Network.
