Job Description:
A company is looking for a Lead Population Health Strategy
Specialist to oversee the design of population health strategy
initiatives and programs.
Key Responsibilities
Leads and oversees the design of complex population health strategy
initiatives to improve care coordination and health outcomes
Monitors care management trends and analytics to support program
initiatives and identify high-risk member populations
Collaborates with senior leadership to provide insights for
improving coordination of care and overall health outcomes
Required Qualifications, Training, and Education
Bachelor's degree required
5 - 7 years of related experience or equivalent experience
Registered Nurse (RN) license required
Experience in chronic conditions/advanced illness clinical
settings
Expertise in designing and developing health programs
