Job Description:
A company is looking for an RN II specializing in Medicaid,
Care, Case, and Disease Management.
Key Responsibilities
Assess patients' clinical needs and ensure appropriate level of
care
Coordinate and implement care plans while monitoring service
delivery
Advocate for members and facilitate communication among
multidisciplinary teams
Required Qualifications
High School Diploma/GED required; Bachelor's degree preferred or
relevant experience in lieu of degree
Minimum of two (2) years clinical experience required
Minimum of two (2) years' experience in health care payer roles
required
Active Unrestricted NJ RN License or active Compact License
required
Proficiency in personal computers and MS Office products
required
