Job Description:
A company is looking for a RN Case Manager to coordinate all
aspects of a patient's care and serve as a patient advocate across
the continuum of care.
Key Responsibilities
Conduct initial intake calls and ongoing care coordination for
complex patients
Develop and implement care plans in collaboration with the
patient's care team
Support patients during care transitions and address gaps in care
to prevent hospital admissions
Required Qualifications, Training, and Education
Current RN License in good standing in the state of practice
Bachelor's degree in nursing or associates in nursing with another
bachelor's degree
Minimum of 4 years of clinical experience, with at least 2 years in
case management
Certified Case Manager (CCM) certification or commitment to
complete when eligible
Geriatric care experience is highly desired
